Diabetes Nurse Service
Service Overview
The Diabetes Nurse Service provides education and support to those living with Diabetes within the Hospital and Community. The aim of the service is to work collaboratively with clients living with Diabetes, along with their whānau, the primary care team, community agencies and hospital services.
Consultations can be carried out at home, at a clinic or hospital setting, or wherever you want to meet. You can expect an assessment, an agreed management plan, ongoing education and follow up as required.
Our specialist Diabetes Nurse is a resource for whānau and clinicians with advanced knowledge around medication and diabetic conditions.
The service also provides up to date treatment options and the most relevant evidence d information and management care plans for whānau.

This Service is Free of Charge for all eligible persons (please see Ministry of Health Eligibility Criteria) for more information
Service Entry
Anyone can make a referral including the individual and/or whānau, GP and Clinical Services, Community Organisations, and Hospital Referrals.
