Kaupapa Māori Advanced Nursing Service

Service Overview

This is a Māori led service, providing tikanga based principles integrated with advanced clinical skills. The Kaupapa Māori Advanced Nursing Service can provide complete client management support, using advanced nursing skills in areas such as;

- Assessment and planning of care for people living with chronic illness(es)
- Close monitoring of health care and responding to client/whānau changing needs
- interpreting and developing individual careplans
- Medication/medicine education
- Assisting client/whānau achieve an optimum level of health
- Advocating/assisting at clinical specialist appointments, at hospital or community appointments
- Referrals to other healthcare services such as podiatry, hospital services etc.

Our approach will include working closely with your GP, Practice Nurse, Hauora, Health Provider, Hospital Services and other Community Organisations. This service is provided in partnership with the client/whānau and is provided in a range of settings such as, at Home, Marae or Community Centre.

Service Coverage Area is from Otamarakau to Otawhiwhi

Service Cost

This Service is Free of Charge for all eligible persons (please see Ministry of Health eligibility criteria) for more information

Service Entry

Anyone can make a referral including the individual and/or whānau, GP and Clinical Services, Community Organisations, and Hospital Referrals.
