REGISTERED & ENROLLED PATIENT FEES
All new adult (18yrs - 64yrs) enrolments are to pay $37.00 when registering with our clinics. The Ministry of Health will require the following identification to verify eligibility (this list is not exclusive)
- NZ Drivers Licence
- NZ Passport
- 18 Card

RE-ENROLLING WITH TE MANU TOROA
We are currently going through the process of re-enrolling ALL patients. This requires filling out a new enrolment form.
Please don’t be offended we require:
Indentification: Drivers License or WINZ Card or Birth Certificate, Gold/Super Card, 2 letters addressed to you at your current address
AND
Eligibility: Passport/or Birth Certificate
