Needs Assessment
& Co-ordination
The Kaupapa Māori Needs Assessment Service Co-ordination (NASC) is a free community service for adults aged between 18 to 65 year. The NASC offers access to a wide range of community-based resilience, recovery and support-focused services.
We work with individuals and their whānau to assess their needs, plan and co-ordinate appropriate services. Needs Assessments are undertaken to identify individual strengths, supports and activities that are likely to lead to resilience and recover outcomes for the individual and their whānau.
Even if they do not make the original referral, your family GP will be kept informed and involved as the primary medical caregiver.
Referrals
Anyone can make a referral, including an individual or their whānau. Referrals to the service can also come from community mental health teams, acute mental health inpatient services, GP’s and other community organisations.
Eligibility
This service is available for adults aged between 18 and 65 years of age living with a mental illness.

Our Home Based Support Service covers the Western Bay of Plenty region
Cost
This service is free of charge for all eligible persons.
