Community Nurse Services

We offer a range of community nursing services targeting critical medical needs within our communities.

Our Community Nursing Services provide complete client management support.

Community Nursing Services include:

  • Assessment and planning of care for people living with chronic illnesses
  • Close monitoring of health care and response to client/whānau changing needs
  • Interpreting and developing individual care plans
  • Medication/medicine education
  • Assisting clients/whānau achieve and optimal level of health
  • Advocating/assisting at clinical specialist appointments at clinics within the hospital or community
  • Referrals to other healthcare services such as podiatry, hospital services etc

All our specialist services are provided in partnership with the client and whānau in a range of settings such as the client’s home, marae or community centre.

Our Services

Complete client management services by our Community Nursing team provide assessment, care and management plans, as well as follow up and support.

Cardiac Services

Cardiac Nursing Services is a community based support service providing education, clinical advice and support to help adults diagnosed with a heart condition. Heart disease describes any type of disorder that affects the heart.

Diabetes Services

Our specialist Diabetes Nurse provides education and support for adolescents and adults living with Type 1 or Type 2 Diabetes. With advanced knowledge around medications and diabetic conditions this service also provides up-to-date treatment options and management care plans for whānau.

Dietitian

Our Dietitian Service offers a free, community based assessment and treatment care plan for nutrition-related health problems such as Diabetes, Obesity and Heart Disease. Personalised Nutrition Care plans with goals are developed after a comprehensive assessment with regular follow up visits for ongoing support.

Respiratory Services

A free community service for children and adolescents with asthma and other respiratory disorders. Our Respiratory Service aims to build on the strengths of children who have been diagnosed with asthma so that early intervention, prevention, treatment and ongoing management is maximised.

Pēpi Ora

Pēpi Ora is a free Well Child Service that is available to all families/whānau for children from birth to five years. Our registered nurses will visit regularly to monitor development as well as providing helpful hints, tips education and advice.

Advanced Nurse Services

To ensure accessibility for all of our clients, our Advanced Nurse Services are delivered in a location that works for you.

This can be at home, work, marae or anywhere that is convenient for you.
