Do you or your whānau have a heart condition?
The Cardiac Specialist Nurse Service is a community based support service for all adults with a heart condition.
Through this service, your or your whānau member will receive education and support from a nursing management prospective to effectively manage the heart condition.
Clinical advice, support and management in regard to chronic heart conditions is delivered in the home, work or clinic that suits.
What is Heart Disease?
Heart Disease is an umbrella term for any type of disorder that affects the heart. Find out more about heart disease on the Heart Foundation website.
Referrals
Anyone can make a referral, including the individual and/or whānau, GP/Clinical Nurse, or Hospital.
Eligibility
This service is available for adolescents and adults living with a heart condition.
Coverage
Our Cardiac Specialist Nurse Service covers the Western Bay of Plenty region.
Cost
This service is free for all eligible persons.
