Te Manu Toroa General Practice clinics provide high quality, affordable and accessible primary health care for their enrolled population. The two clinics are based at Tauranga Moana City Clinic (Tauranga) and Te Akau Hauora (Pāpāmoa).
Te Manu Toroa is the largest Kaupapa Māori provider of Primary Health Services in the Western Bay of Plenty. Our practice teams are dedicated to providing you and your whānau with comprehensive healthcare services.

Practice Plus
Practice Plus is our trusted virtual health provider, available weekdays until 10pm and weekends and public holidays from 8am to 8pm.
All Practice Plus Clinicians are experienced and licensed practitioners registered with the Medical Council of New Zealand or the Nursing Council of New Zealand.

Hei Oranga
Hei Oranga, meaning to have wellbeing, is the primary mental health and wellbeing service within Te Manu Toroa. Kaimahi support whanau of all ages enrolled in the TMT GP Clinics who may require awhi tautoko for their health and wellbeing. The team is made up of clinical and non-clinical Kaimahi who work therapeutically towards whanau led wellbeing goals. Hei Oranga Kaimahi can provide regular therapeutic support or on the day brief intervention depending on the need of the whanau. Appointments are free and can be made through the Clinic, alongside your GP/Nurse or as a self-referral (see our contact us tab).
The Hei Oranga Team is made up of:
- Wellbeing Clinician Clinical role providing therapeutic intervention (1 hour) of up to 6 sessions for whānau presenting with mild – moderate mental health needs. This role works with youth (10-18yrs) and adults (19+). Whānau require referral to access this service.
- Health Improvement Practitioner (HIP) Clinical role delivering immediate, on the day, brief behavioural intervention (30mins max) to support mental health and wellbeing. There is no age criteria or maximum number of visits. Whānau can book directly through the GP clinic the same as any other medical appointment.
- Health Coaches Non-clinical role that works collaboratively with whānau towards their wellbeing goals. This encompasses all areas of hauora not just mental health and can be delivered within the GP clinic, community settings and groups such as fitness. Whānau can book directly through the GP clinic the same as any other medical appoinment.
Immunisations
Help us to help protect your children by bringing them in on time for their immunisations. Immunisations are due at 6 weeks, 3 months, 5 months, 15 months and 4 years old.

We are required by the Ministry of Health to ensure that all of our tamariki are fully immunised according to the national immunisation schedule. This may mean that our staff will try any way possible to make contact with you. It is crucial that your contact details are current to enable us to contact you when immunisations are due.
Mobile Immunisation
Mobile Immunisation Nurses are available to immunise in homes, Kōhanga Reo or where ever is more appropriate for the Tamaiti
Care Plus
Care Plus funding is provided to general practices to improve chronic care management by reducing the cost of services for people that need to see a doctor or nurse often.
If you are eligible to be in the Careplus program, you will have an initial free appointment with your GP and Nurse and together a health plan will be developed.
You will receive 3 follow up appointments free of charge so the plan can be reviewed and changed as necessary.

- Eligibility Able to benefit from intensive clinical management

- Time Limited to four fully funded doctor or nurse visits every 12 months
Diabetes Care Improvement Package
Intensive clinical care for people with diabetes that have a high blood sugar level.
Diabetes free checks pick up any changes in the patient’s well being early enough to limit or avoid further complications. It is important these are done yearly.
- Entry Criteria Diagnosed with diabetes
- Eligibility Has a HbA1c blood sugar level > than 75 mmol/mol
- Age Range All persons
- Referral Ask doctor or nurse
- Time Eight fully funded GP/Nurse visits over 12 months
Dental Services
We provide Dental Services specifically for pēpi, tamariki and rangatahi up to 18 years of age. These services are delivered out of two specialised mobile units that are located at many of our kōhanga reo, kura kaupapa Māori and colleges throughout Tauranga and Western Bay of Plenty.
We also have a fully equipped fixed surgery clinic located in Papamoa.

B4 School Check
The B4 School Check is a free health and development check for your 4-year-old. It’s the last Well Child Tamariki Ora check and helps give your child the best start at school.
- Entry Criteria Enrolled children
- Age Range Soon after a child’s 4th birthday
- Referral Parent booking or B4SC Coordinator will contact parent/caregiver
Cervical Screening
Regular cervical smear tests are a women’s best protection against developing cervical cancer.
- Entry Criteria Female
- Eligibility Free to all Maori women enrolled at a Te Manu Toroa clinic
- Age Range 25 to 70 years of age
Patient portals
Patient portals are secure online sites where patients can access their health information and interact with their general practice to make appointments, request prescriptions or view medical records.

The Ministry of Health will require the following identification to verify eligibility (this list is not exclusive).
